Why do I need to have this information? You will need the information to access your UHC member account at www.myuhc.com.

Schedule your appointment to ensure you meet with an FBMC Representative at a convenient time and date.

Keeping you safe! Offering you multiple ways to enroll with an FBMC Representative:

• Select Call Center as the location and a Benefits Counselor will call you at your scheduled appointment time (the call will originate from area code 850)

• Select Virtual Enrollment Session (VES), you will receive a follow up phone call from a Benefits Counselor, who will provide you a link to join your virtual session. You must prepare to have your session in front of a computer (no cell phones or tablets).

The FBMC Representative will assist you with the following during your one-on-one (virtual or telephonic) session:

• Explain changes to the 2021 Health Rewards program, overall requirements, and content in the Health Rewards planner.
• Provide an overview of the district's NEW onsite clinic.
• Explain how Ocenture's ID Commander Identity Theft Plan can help protect your family and your future.
• Explain how Trustmark's Voluntary Benefits can help provide protection against life's ups and downs.
• Learn how to access "ALEX" year-round.
• Discuss IRS 1095 reporting.
• Enter you into the daily drawing.
